Lancashire County Council is seeking an experienced and dedicated Adult Social Worker to join our Integrated Care Community Team, based in Lancaster and surrounding areas. In this role, you will play a key part in supporting timely and safe hospital discharge, conducting triage and duty work, and carrying out assessments to ensure individuals receive the right care and support.

Key Responsibilities:
  • Hospital Discharge: Ensuring timely and safe transitions from hospital to home or community, with necessary support in place.
  • Triage/Duty Work: Responding to urgent cases, assessing needs, and prioritizing support to provide the right level of care.
  • Continuing Healthcare (CHC) MDT: Collaborating with multi-disciplinary teams to assess eligibility for Continuing Healthcare funding and develop effective care plans.
  • Discharge to Assess (D2A) Reviews: Conducting D2A reviews to assess individuals' ongoing care needs at home or in care settings, ensuring appropriate support.
  • Collaboration: Working closely with health professionals, carers, and families to ensure a coordinated approach to care that promotes individual well-being and independence.

We are looking for candidates who possess:

  • A social work qualification and registration with Social Work England.
  • An enhanced DBS disclosure.
  • Experience working with adults, particularly in hospital discharge, triage, or Continuing Healthcare processes.
  • The ability to balance office-based work with community visits.

The estimated annual salary for this position is £32,000-£42,000.
