Job Overview

We are seeking a highly skilled and motivated Specialist Community Public Health Nurse to join our team in Bolton. As a key member of our integrated 0-19/25 neighbourhood teams, you will work collaboratively with other professionals to deliver high-quality health and wellbeing services to children, young people, and families.

Key Responsibilities:

  • Manage a caseload of universal plus and partnership plus cases, ensuring effective collaboration with other agencies to promote child and family wellbeing.
  • Lead on the delivery of The Healthy Child Programme, providing comprehensive health and development reviews, health promotion, screening, and support tailored to individual needs.
  • Work closely with other professionals to identify and address potential risks and challenges affecting child and family health.

Requirements:

  • UK NMC registration
  • SCPHN qualification in practice
  • Proven ability to work autonomously and collaboratively as part of a team
