Job Overview

We are seeking a skilled and compassionate Care Coordinator to join our residential service. As a key member of our team, you will play a vital role in supporting individuals experiencing mental health challenges to develop the skills and confidence needed to maintain their independence and improve their quality of life.

About the Role

The successful candidate will be responsible for:

  • Developing personalized support plans that cater to the unique needs and goals of each individual
  • Providing guidance and support to help individuals manage their daily lives, including personal care, medication management, and social activities
  • Maintaining accurate records of individual progress and reporting any concerns or incidents to relevant authorities
  • Collaborating with other healthcare professionals and external agencies to ensure comprehensive care
  • Conducting regular reviews and assessments to adjust support plans as needed
Requirements

To be considered for this rewarding role, you will need:

  • A degree in Social Work, Psychology, or a related field
  • Proven experience working with vulnerable adults, preferably in a mental health setting
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with individuals from diverse backgrounds
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage competing demands
  • A commitment to delivering person-centered care and promoting positive outcomes for individuals
