We are seeking an experienced Clinical Rehabilitation Specialist to join our Dartford PCN Team, based at Livingstone Hospital. As a Band 4 Associate Practitioner, you will play a vital role in delivering high-quality patient care and contributing to the development of our team.

  • Salary: £28,000 - £34,000 per annum, depending on experience
  • Job Type: Full-time
  • Location: Dartford, Kent

This is an ideal opportunity for someone who has previous experience working as a band 3 rehabilitation/therapy assistant in an NHS community rehabilitation setting. You will possess the ability to operate community equipment, assess needs and implement care, and work well as a team but also be able to organise your own workload without direct supervision.

Due to the travel included in this role, a Full UK driving license with access to a car is needed to carry out the duties of the post.
