About the Role

We are seeking an experienced and passionate Mental Health Community Care Navigator to join our Community Mental Health Team.

As a key member of our team, you will work alongside mental health and primary care professionals to support individuals with personalized support, addressing the broader social determinants of health and wellbeing.

Key responsibilities include:

  • Supporting mental health service users with continuation of care and support
  • Linking individuals to appropriate community health, social care, and voluntary sector services and support networks
  • Reducing the risk of social exclusion or mental health relapse
  • Directing individuals into specialist mental health services where appropriate

What You Will Achieve

As a Mental Health Community Care Navigator, you will make a significant difference in the lives of individuals and communities. You will:

  • Proactively engage with service users, identifying non-clinical needs and providing relevant support and guidance
  • Work collaboratively with mental health and primary care professionals to deliver great outcomes for service users
  • Contribute to a multi-disciplinary team, sharing expertise and best practices to improve services
