Job Summary

We are seeking an experienced Mental Health Practitioner to join our integrated Access Team. As an Access Team Specialist, you will play a pivotal role in improving the lives of individuals using our services, supporting them to access the most appropriate care for their needs.

Main Responsibilities
  • Develop and implement holistic, biopsychosocial interventions to support individuals with complex mental health conditions.
  • Assess and manage risk, developing clear care pathways to reduce the risk of further crisis developing.
  • Provide high-quality care in a variety of settings, including acute and community facilities.
  • Liaise with community-based services and resources to facilitate the delivery of the most appropriate care for individuals and their families.
  • Discuss complex issues surrounding risk management and plan care accordingly.
  • Discharge planning and respond to client crisis in a non-judgmental, positive, and proactive manner.
Person Specification

We are looking for an individual with:

  • A current professional qualification (UK RMN/RNMH/SW or equivalent) and evidence of postgraduate training/studies.
  • Two years' post-registration level experience working with mental illness of a severe and enduring nature in an acute or crisis setting.
  • Proven and advanced skills in prioritizing workload, risk assessment, communication, and clinical abilities.
