About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our integrated Access Team at Navigo Health and Social Care CIC. As an Access Team Practitioner, you will play a pivotal part in improving the lives of individuals using our services, supporting them to access the most appropriate services for their needs.

Main Responsibilities

The successful candidate will be responsible for initial assessment, implementation, planning, and evaluation of care. This will include assessment of risk, subsequent management, and safety planning, as well as face-to-face and over-the-telephone interventions. You will work within a 24/7 rota, providing essential mental health assessment and support, and will be responsible for caseload management from first contact to discharge.

Key Skills and Qualifications

To be successful in this role, you will need to have a current professional qualification (UK RMN/RNMH/SW or equivalent) and evidence of postgraduate training/studies. You will also need to have two years' post-registration level experience, with two years' experience working with mental illness of a severe and enduring nature in an acute or crisis setting. Proven skills in risk assessment, communication, and liaison with a range of others are essential.
