We are seeking an experienced and passionate Mental Health Community Care Navigator to join our Community Mental Health Team at Rethink Mental Illness.
Key Responsibilities:
- Support mental health service users with personalised care and support to address the broader social determinants of health and wellbeing.
- Link individuals to appropriate community health, social care, and voluntary sector services and support networks to reduce the risk of social exclusion or mental health relapse.
- Direct people into specialist mental health services where appropriate and support individuals throughout their journey.
- Work alongside mental health and primary care professionals to deliver great outcomes for people using services.
What You'll Bring:
- Experience in mental health community care or a related field.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Passion for delivering high-quality care and support to individuals with mental health needs.
