About the Role

We are seeking a highly motivated and compassionate Mental Health Support Worker to join our team at Navigo Health and Social Care CIC. As a key member of our Acute Older Peoples Mental Health Service, you will play a vital role in delivering individual treatment plans and providing holistic care to our service users.

Key Responsibilities
  • Deliver individual treatment plans and provide holistic care to service users
  • Work closely with Care Coordinators through the care programme approach and care management process
  • Provide support and assistance with daily living activities, including personal care
  • Facilitate community and hospital appointments as necessary
  • Monitor service users' progress, level of functioning, and mental state, and alert the appropriate staff involved in their care
  • Assist the Care Co-ordinator/Key Worker and service user in developing crisis/contingency plans and advanced statements
  • Maintain adequate records, as required, by existing procedures
Person Specification
  • NVQ Level III or Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience
  • Experience of working with people with Mental Health problems
  • An interest in and commitment to on-going personal/professional development
  • Basic IT knowledge
