Mental Health Support Worker

We are seeking a highly motivated and compassionate Support Worker to join our Acute Older Peoples Mental Health Service team at Navigo Health and Social Care CIC. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with mental health needs, promoting their independence and well-being.
Key Responsibilities- Deliver individual treatment plans across boundaries of care, working closely with Care Coordinators through the care programme approach and care management process.
- Provide support, give time to service users within the unit and the community, and promote recovery.
- Assist Care Coordinators or key workers and service users to assess, plan, implement, and evaluate individual care plans and undertake other assessments as required.
- Work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis, and take charge of an area in the absence of qualified staff.
- Ensure service users engage effectively with the agreed care plan and across appropriate services provided on a regular and consistent basis.
- Plan and supervise individual or group therapy sessions, in line with the agreed care plan or programme of care.
- Provide regular and practical support to service users and their carers/significant others in developing and managing dignity and independence.
- NVQ Level III or Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience.
- Experience of working with people with Mental Health problems.
- An interest in and commitment to on-going personal/professional development.
- Basic IT knowledge.
