About the Role

We are seeking an experienced and passionate Mental Health Community Care Navigator to join our Community Mental Health Team.

This is an exciting opportunity to work as part of a multi-disciplinary team, supporting individuals with personalised care and addressing the broader social determinants of health and wellbeing.

Key Responsibilities
  • Support mental health service users with continuation of care and support to address social determinants of health and wellbeing.
  • Link individuals to appropriate community health, social care, and voluntary sector services and support networks.
  • Direct people into specialist mental health services where appropriate and support individuals throughout their journey.
  • Proactively engage with service users, identify non-clinical needs, and provide relevant support and guidance to improve mental wellbeing and quality of life.
