We are seeking an experienced and passionate Mental Health Community Care Navigator to join our Community Mental Health Team.
This is an exciting opportunity to work as part of a multi-disciplinary team, supporting individuals with personalised care and addressing the broader social determinants of health and wellbeing.
Key Responsibilities- Support mental health service users with continuation of care and support to address social determinants of health and wellbeing.
- Link individuals to appropriate community health, social care, and voluntary sector services and support networks.
- Direct people into specialist mental health services where appropriate and support individuals throughout their journey.
- Proactively engage with service users, identify non-clinical needs, and provide relevant support and guidance to improve mental wellbeing and quality of life.
