About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Integrated Access Team at Navigo Health and Social Care CIC. As an Access Team Practitioner, you will play a pivotal part in improving the lives of individuals using our services, supporting them to access the most appropriate services for their needs.

Key Responsibilities
  • Develop and implement holistic, biopsychosocial interventions to support individuals with complex mental health needs.
  • Assess and manage risk, developing clear care pathways to reduce the risk of further crisis developing.
  • Provide high-quality care in a variety of settings, including acute and community facilities, and in the community.
  • Liaise with community-based services and resources to facilitate the delivery of the most appropriate and best possible care for individuals, carers, and their families.
  • Discuss complex issues surrounding risk management and plan care accordingly, keeping relevant people informed.
  • Discharge planning and respond to client crisis in a non-judgemental, positive, and proactive manner.
Person Specification
  • A current professional qualification (UK RMN/RNMH/SW or equivalent) and evidence of postgraduate training/studies.
  • Two years' post-registration level experience working with mental illness of a severe and enduring nature in an acute or crisis setting.
  • Knowledge of relapse prevention and psychosocial interventions.
  • Proven and advanced skills in prioritising workload, risk assessment, communication, and clinical abilities.
