Job Summary

We are seeking an experienced Access Team Practitioner to join our integrated Access Team. As a key member of our team, you will be responsible for providing mental health assessment and support to individuals in crisis. You will work closely with our multidisciplinary team to develop and implement care plans that promote recovery and well-being.

Main Responsibilities
  • Conduct comprehensive mental health assessments to identify individual needs and develop personalized care plans.
  • Provide crisis intervention and support to individuals in need, including face-to-face and telephone support.
  • Work collaboratively with our multidisciplinary team to develop and implement care plans that promote recovery and well-being.
  • Supervise and mentor junior colleagues to ensure high-quality care and support.
  • Participate in quality monitoring and improvement activities to ensure our services meet the highest standards.
Requirements
  • Current professional qualification in mental health nursing or a related field.
  • Postgraduate training or studies in mental health or a related field.
  • At least two years' experience working in mental health, with a focus on crisis intervention and support.
  • Proven skills in risk assessment, communication, and liaison with other professionals.
  • Ability to work flexibly and prioritize workload to meet the needs of individuals in crisis.
