About the Role

We are seeking a dedicated Social Services Practitioner to join our team at Tameside Council. As a key member of our adult services team, you will be responsible for supporting individuals to live well at home.

Key Responsibilities

  • Manage a caseload of adult service users, providing tailored support to enhance wellbeing and independence.
  • Conduct Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions.
  • Collaborate with internal and external services to provide a coordinated response to individuals and families.
  • Maintain professional relationships with other disciplines and local health and social care services.

About You

To be successful in this role, you will require excellent communication skills and the ability to build effective professional relationships.

Essential Criteria

  • A current valid driving licence and access to a vehicle for work use.
