About the Role

We are seeking a dedicated Social Services Practitioner to join our team at Tameside Council. As a key member of our Adult Services team, you will play a vital role in supporting adults to live well at home.

Key Responsibilities:

  • Manage a caseload of adult clients, providing high-quality support and guidance to ensure their needs are met.
  • Conduct Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions to ensure individuals receive the necessary care and support.
  • Work collaboratively with other local health and social care services to provide a coordinated response to individuals and families.
  • Develop and maintain effective professional relationships with colleagues, stakeholders, and service users.

Requirements:

  • Excellent communication and interpersonal skills.
  • Ability to build trust and rapport with service users, families, and professionals.
  • Strong assessment and planning skills, with the ability to analyze complex information and make informed decisions.
  • Knowledge of relevant legislation, policies, and procedures.
  • A valid driving license and access to a vehicle for work use.
