Diabetes Care Coordinator

We are seeking a highly motivated Registered Nurse (level 1) with excellent communication and leadership skills to join our multidisciplinary Diabetes Team at Scarborough Hospital. The successful candidate will have a proven interest in diabetes and diabetes care.
The post holder will support the co-ordination of in-patient diabetes care, ensuring effective communication at all times, providing support to qualified and unqualified nurses within the ward/department.
Main Duties and ResponsibilitiesThe job holder will have a Trust-wide responsibility to promote clinical excellence, working jointly with the current lead In-Patient Diabetes Specialist Nurse to assist in delivering and developing the in-patient diabetes service.
They will assess the needs of inpatients with diabetes across the Trust, identifying potential and actual problems, and implementing an effective care plan.
The post holder will also provide support to Healthcare Professionals in the community to facilitate a smooth discharge.

Post registration qualification in specialty
Non-medical prescriber - as determined by the needs of the service
