Adult Dementia Care Coordinator

Job Summary: This is an exciting opportunity for a qualified RGN/RMN to join our community team in Scarborough CMHT. As a Band 6 Community Psychiatric Nurse, you will have the chance to work with a diverse range of patients and develop your skills in a supportive environment.
Duties and Responsibilities:
- Assess and manage patients' mental health needs
- Develop and implement individualised care plans
- Work collaboratively with other healthcare professionals to achieve positive patient outcomes
Qualifications and Experience:
- RGN/RMN qualification with current PIN number
- Fully enhanced DBS check
- Experience working in a community setting
