Nursing Care Specialist

We are seeking an experienced Nursing Care Specialist to join our team in providing high-quality care and support to patients in their own homes. This is a unique opportunity to make a real difference in the lives of others.
Key Responsibilities- Assess patient's condition through observation and clinical skills, recording appropriately on EMIS and ensuring the relevant professionals are informed in a timely manner.
- Provide care and rehabilitation for service users within their own homes under the clinical direction of qualified staff.
- Support multi-disciplinary health professionals in providing a comprehensive rehabilitation programme to prevent avoidable hospital admission.
This role offers a competitive salary of £25,000 - £31,000 per annum, depending on experience, plus benefits including opportunities for career progression and full training provided.
The successful candidate will be required to work shifts of 8am - 4pm and/or 2pm - 10pm, across 7 days a week, with full and part-time roles available.
