Job SummaryThis Band 5 nursing opportunity involves working on Chartridge Ward, Waterside Unit, where we provide inpatient rehabilitation for older adults. Our patients often have multiple health needs and chronic conditions, requiring a compassionate and skilled approach to care.
Main Responsibilities- Assess the nursing needs of individual patients and develop personalized care plans.
- Collaborate with multidisciplinary teams to deliver effective patient care.
- Supervise and mentor junior staff members to ensure optimal care delivery.
- Evaluate patient outcomes and modify care plans accordingly.
