Senior Clinical Lead

An exciting opportunity has arisen within the Children and Young People's Continuing Care Team at Wirral University Teaching Hospitals NHS Trust. The service offers home-based care and community support to clients with complex medical needs.
As a Senior Clinical Lead, you will be involved in the clinical supervision and ongoing support of our team of Senior Clinical Support Workers and Band 5 nurse colleagues. The role will involve supervision, training, and therefore requires a good level of clinical experience.
Responsibilities include planning, implementation, and evaluating care in conjunction with parents, carers, and other allied health, education, and social care professionals. You will be working with children and young people with various levels of dependency, including those with complex health issues and severe disabilities.
Main Duties:
- Identify children with complex health needs and undertake assessments under the guidance of the National Framework for Children and Young People's Continuing Care.
- Work in collaboration with children and young people and families to understand their preferences regarding care.
- Lead on holistic assessments of the child/young person and the family.
- Act as the Lead Health professional to correlate reports/risk assessments from the MDT in respect of bespoke packages of care.
- Convene and lead Multi-Disciplinary Meetings to plan care and ensure the needs of vulnerable children/families are met.
- Attend Children's Commissioning panel and make recommendations around funding arrangements.
- Participate in safeguarding meetings in accordance with Trust and local Safeguarding Children's procedures, policies, and guidance.
- Be actively involved in the training process for parents, carers, and other professionals to ensure competency and evidence-based practice.
- Participate in common assessment framework CAF/Team Around the Child TAC process following integrated working guidelines.

Person Specification:
Qualification:
- RSCN / RN child/ LD Nurse
- Minimum of 3 years post-registration experience
Knowledge and Skills:
- Sound working knowledge of Child Protection
Personal Attributes:
- Ability to organise, prioritise and plan own caseload
- Excellent communication skills, verbal and written
- To be able to recognise the workload of colleagues and support as appropriate
- Ability to work independently and as part of a team
- Ability to deal with conflict and sensitive situations
- In conjunction with complex care manager, identify own learning needs
