Job Summary

We are seeking a skilled and compassionate Band 5 Mental Health Nurse to join our team in a development post. As a Community Psychiatric Nurse, you will work with a diverse and multicultural team to provide specialist mental health care to individuals with complex needs.

This role offers a unique opportunity to work in a dynamic and varied environment, covering depot and clozapine clinics, home depots, and duty. You will be responsible for conducting comprehensive nursing assessments, working collaboratively with other disciplines, and delivering patient-centered care to support recovery.

Key Responsibilities
  • Conduct thorough nursing assessments, taking into account mental, physical, social, cultural, psychological, spiritual, and environmental factors.
  • Work collaboratively with patients, carers, and other disciplines to develop holistic care plans.
  • Deliver safe and competent patient-centered care, promoting recovery and well-being.
  • Adhere to NMC registration and revalidation requirements, as well as KMPT's policies and procedures.
  • Assess, act on, and report safeguarding concerns.
  • Optimize health and wellbeing, promoting self-care and involving carers where appropriate.
  • Carry out specific duties relevant to community care medicines clinics and home visit depots.
  • Record and report changes in patient behavior and risks using redboard meetings and approved KMPT systems.

Person Specification
  • Current NMC registration
  • Knowledge and ability to apply relevant legislative frameworks and policies
  • Previous experience of working in adult mental health and within the NHS
Clinical Knowledge
  • Sound understanding of secondary mental health diagnoses
  • Completion of Edward Jenner Programme
  • Good understanding of Physical Health conditions
  • Understanding of interventions offered in CMHT
  • Previous experience of working in clozapine/depot clinics
Mental Health Knowledge
  • Understanding of the Mental Health Act 1983 amended 2007
  • Understanding of the Mental Capacity Act
  • Understanding of cognitive behavioural therapy
Disclosure and Barring Service Check
