Job Title: Band 5 Mental Health Services Nurse

We are seeking a skilled and compassionate Band 5 Mental Health Services Nurse to join our team at Kent & Medway NHS & Social Care Partnership Trust. As a key member of our community mental health team, you will play a vital role in providing high-quality care to individuals with complex mental health needs.

Job Summary

This is a development post that offers the opportunity to work in a dynamic and diverse team, providing specialist mental health care to individuals with multiple and complex needs. You will work collaboratively with other disciplines, patients, and their carers to agree a holistic person-centred care plan that addresses the needs identified through assessment.

Key Responsibilities
  • Conduct comprehensive, systematic nursing assessments that take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors.
  • Work collaboratively with other disciplines, patients, and their carers to agree a holistic person-centred care plan.
  • Deliver and evaluate safe, competent patient-centred care in partnership with the service user and their carers to support recovery.
  • Adhere to the NMC registration and revalidation requirements.
  • Assess, act on, and report any safeguarding concerns.
  • Optimise health and wellbeing by promoting self-care and involving carers where appropriate.
Requirements
  • Current NMC registration.
  • Knowledge and ability to apply relevant legislative frameworks and policies relating to care, e.g. Mental Capacity Act.
