Mental Health Community Nurse

We are seeking a skilled and compassionate Mental Health Community Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals with complex mental health needs.
Key Responsibilities- Conduct comprehensive mental health needs assessments, developing and reviewing care plans and community care packages in line with agreed health and social care eligibility criteria.
- Provide assessments and interventions to mental health service users, including assessment, planning, implementation, and evaluation of treatment plans.
- Work collaboratively with the multi-disciplinary team, Primary Care, voluntary agencies, and other stakeholders to ensure seamless care delivery.
- Ensure that care is evidence-based, in line with NICE guidelines, and sensitive to the needs of all service users.
- Participate in rotas as required and provide clinical cover for team members in their absence.
- Liaise with CRHTT and inpatient services as needed, providing written reports and attending Mental Health Review Tribunals and Care Planning Meetings.
- Holds a qualification in mental health nursing.
- Has experience working with individuals with complex mental health needs.
- Can work creatively and flexibly.
