About the Role

We are seeking a skilled and compassionate Mental Health Community Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities
  • Conduct comprehensive mental health needs assessments and develop individualized care plans in collaboration with service users, carers, and other healthcare professionals.
  • Provide evidence-based medical and psychosocial interventions to service users, ensuring that care is tailored to their unique needs and preferences.
  • Work collaboratively with the multi-disciplinary team, including primary care, voluntary agencies, and other stakeholders to ensure seamless care coordination and effective service delivery.
  • Participate in rotas and provide clinical cover for team members as required, ensuring that service users receive consistent and high-quality care.
  • Contribute to the development and implementation of trust policies and procedures, ensuring that they are aligned with best practice and meet the needs of service users.
Requirements
  • Hold a qualification in mental health nursing and have relevant experience working with individuals with complex mental health needs.
  • Be able to work creatively and flexibly, adapting to changing circumstances and priorities.
  • Have excellent communication and interpersonal skills, with the ability to build strong relationships with service users, carers, and other healthcare professionals.
  • Be committed to ongoing professional development and continuous learning, staying up-to-date with the latest research, policies, and best practices in mental health care.
