Job Title: Band 5/6 Mental Health Community Nurse

We are seeking a skilled and compassionate Mental Health Community Nurse to join our team at Kent & Medway NHS & Social Care Partnership Trust. As a key member of our community mental health team, you will play a vital role in providing high-quality care to individuals with complex mental health needs.

About the Role:
  • Conduct comprehensive mental health needs assessments and develop care plans in collaboration with service users, families, and multidisciplinary teams.
  • Provide evidence-based medical and psychosocial interventions to support service users in achieving their recovery goals.
  • Work collaboratively with community teams, primary care, and voluntary agencies to ensure seamless care coordination.
  • Participate in rota's and provide clinical cover for team members as required.
Requirements:
  • Qualification in mental health nursing and experience working with individuals with complex mental health needs.
  • Ability to work creatively and flexibly in a dynamic environment.
  • Strong communication and interpersonal skills to build effective relationships with service users, families, and colleagues.
