Mental Health Nurse

We are seeking a skilled and compassionate Band 5 Mental Health Nurse to join our diverse and multi-cultural team at Thanet CMHT. As a development post, this role offers an exciting opportunity to work in a dynamic environment, providing specialist mental health care to individuals with complex needs.
Key Responsibilities- Conduct comprehensive assessments of patients, taking into account their mental, physical, social, cultural, psychological, spiritual, and environmental factors.
- Work collaboratively with other disciplines, patients, and their carers to develop holistic care plans that address identified needs.
- Deliver safe and competent patient-centred care, supporting recovery and promoting self-care.
- Adhere to NMC registration and revalidation requirements, as well as KMPT's policies and procedures.
- Assess and report safeguarding concerns, optimising health and wellbeing for service users.
- Carry out specific duties related to community care medicines clinics and home visit depots.
- Record and report changes in patient behaviour and risks, using redboard meetings and approved KMPT systems.
- Promote a healthy, safe, and secure environment, maintaining confidentiality and adhering to Trust policies.
