We are seeking an experienced and motivated Occupational Therapist to join our dynamic Community Service team. As a Senior Occupational Therapist, you will play a key role in providing high-quality patient-centered care and contributing to the development of our service.
Main Responsibilities:- Work closely with a friendly team of physiotherapists, occupational therapists, nurses, OT assistants, and integrated healthcare assistants to maintain independence and avoid hospital admission.
- Provide rapid response to patients in their own home who are in physical health crisis and at risk of hospital admission.
- Continuously prioritize caseload to be responsive to changing daily needs of service users, communicating closely with other service providers to avoid admission where safe to do so.
- Develop and implement individualized care plans to promote patient independence and well-being.
- Experience of working at B5/B6 level.
- Experience of previous NHS work.
- Degree in Occupational Therapy.
- HCPC registration.
