Job Overview

We are seeking an experienced and motivated Occupational Therapist to join our innovative and dynamic community service. As a Senior Occupational Therapist, you will be working closely with a friendly team of physiotherapists, occupational therapists, nurses, rehabilitation assistants, and integrated healthcare assistants who are committed to maintaining independence, promoting self-care, and assisting patients to achieve their goals.

Main Duties of the Job

The Rehabilitation and Frailty Team (RAFT) provides daily therapy in the patient's own home. You will be responsible for prioritizing our caseload to be responsive to the changing daily needs of our service users, communicating closely with other service providers to avoid admission where it is safe to do so, and providing onward referrals to other community services including health and social care.

Key Responsibilities

  • Assess and develop individualized care plans to promote patient independence and self-care
  • Collaborate with multidisciplinary teams to provide comprehensive care and support
  • Conduct home visits to deliver therapy and provide support to patients and their families
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery

Requirements

To be successful in this role, you will need:

  • Experience of working at B5/B6 level
  • Experience of previous NHS work
  • Degree in Occupational Therapy
  • HCPC registration
