**Job Summary:**
We are seeking a highly organised and enthusiastic individual to join our team as our new Care Coordinator. This role will provide essential administrative support to the nursing and dementia care teams, ensuring a seamless transition for new residents and maintaining excellent service for current residents.
The successful candidate will be responsible for providing professional, welcoming, customer-focused service to all residents, visitors, and staff, following relevant policies and procedures to ensure their safety and wellbeing.
**Key Responsibilities:**
- Providing administrative support to the nursing and dementia care teams
- Ensuring a smooth transition for new residents
- Maintaining excellent service for current residents

**Estimated Salary:** £28,000 - £32,000 per annum
