As a General Health Practitioner, you will be responsible for providing high-quality patient care and working closely with our multidisciplinary team to deliver excellent healthcare services.

Key Responsibilities:

  • Undertake a variety of duties including surgery consultations, telephone consultations, and visiting patients at home.
  • Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness.
  • Develop care plans for health in consultation with patients and in line with current Practice disease management protocols.
  • Provide counselling and health education.
  • Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
  • Record clear and contemporaneous consultation notes to agreed standards.
  • Collect data for audit purposes and comply with QOF requirements.
  • Deliver enhanced services.
  • Prescribe in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.

Other Responsibilities:

  • Awareness of and compliance with all relevant PCN and Practice policies/guidelines.
  • A commitment to life-long learning and audit to ensure evidence-based best practice.
  • Contributing to evaluation/audit and clinical standard setting within the organisation.
  • Contributing to the development of computer-based patient records.
  • Contributing to the summarising of patient records and Read-Coding patient data.
  • Attending training and events organised by the Practice or other agencies, where appropriate.

Requirements:

  • Highly skilled and dedicated Primary Care Physician with excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritise tasks accordingly.
  • Strong clinical skills and knowledge of current healthcare practices and guidelines.
  • Ability to work collaboratively as part of a multidisciplinary team.
