Job Summary: We are seeking a highly skilled and dedicated Primary Care Physician to join our team at Conexus Healthcare CIC. As a General Health Practitioner, you will be responsible for providing high-quality patient care and working closely with our multidisciplinary team to deliver excellent healthcare services.
Key Responsibilities:
- Undertake a variety of duties including surgery consultations, telephone consultations, and visiting patients at home.
- Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
- Screen patients for disease risk factors and early signs of illness.
- Develop care plans for health in consultation with patients and in line with current Practice disease management protocols.
- Provide counselling and health education.
- Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
- Record clear and contemporaneous consultation notes to agreed standards.
- Collect data for audit purposes and comply with QOF requirements.
- Deliver enhanced services.
- Prescribe in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
Other Responsibilities:
- Awareness of and compliance with all relevant PCN and Practice policies/guidelines.
- A commitment to life-long learning and audit to ensure evidence-based best practice.
- Contributing to evaluation/audit and clinical standard setting within the organisation.
- Contributing to the development of computer-based patient records.
- Contributing to the summarising of patient records and Read-Coding patient data.
- Attending training and events organised by the Practice or other agencies, where appropriate.
Requirements:
- Highly skilled and dedicated Primary Care Physician with excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritise tasks accordingly.
- Strong clinical skills and knowledge of current healthcare practices and guidelines.
- Ability to work collaboratively as part of a multidisciplinary team.
