The NHS is seeking a highly skilled Meds Manager to join our team in Wakefield. As a key member of our primary care network, you will play a crucial role in optimising the quality of prescribing across our healthcare services.

Key Responsibilities:

  • Develop and manage medicines management plans, delivering patient services as per practice/PCN policy and local/national guidelines.
  • Lead the development and management of the medicines management team, focusing on cost-effective prescribing and improved patient care.
  • Undertake medication reviews for patients with polypharmacy, particularly older adults, those in residential care homes, and individuals with multiple comorbidities.
  • Provide leadership on quality improvement and clinical audit initiatives, managing aspects of the Quality and Outcomes Framework (QOF).
  • Deliver medication review services via clinics, domiciliary visits, and in residential and nursing homes.
  • Produce patient information leaflets, posters, and support medicine awareness projects throughout the year.
  • Promote evidence-based resources and information to support the medicine management team and relevant healthcare professionals.

About You:

  • Relevant experience working at a specialist level with CCGs, community/hospital pharmacy services, or at primary care interfaces.
  • Demonstrable awareness and understanding of risk stratification approaches to identify chronic disease and long-term condition patients.
  • Awareness and experience of systems supporting patient management in primary care settings, delivering pharmaceutical input and support within pathways of care and organisational business.
  • Experience contributing to clinical audit and quality initiatives.
  • Skilled in innovative training and supervision methods.
  • Masters degree in Pharmacy or equivalent.
  • Ongoing professional development and application to practice within the last 12 months.

We estimate the salary for this position to be around £55,000 - £65,000 per annum, commensurate with experience.
