Pharmacist - Medication Specialist

About the Role
HealthNet Homecare is seeking a highly skilled Pharmacist to join our operations department on a full-time, permanent basis.
The successful candidate will be responsible for ensuring the safe and effective delivery of medication to patients in their own homes.
Main Responsibilities
- Undertake the role of the Responsible Pharmacist as required, ensuring correct records are maintained.
- Follow and promote relevant SOPs and Company policy and procedure.
- Carry out clinical screening of prescriptions, ensuring appropriateness for the patient.
- Provide counselling and advice to patients by telephone, email, or in person as required.
- Record interventions made on the electronic patient record as required by the relevant Standard Operating Procedures.
- Understand pharmacovigilance regulatory and contractual requirements.
- Undertake final accuracy checking efficiently and accurately when required to do so.
- Ensure that the HealthNet Homecare pharmacy operation meets all regulatory, contractual, agreed procedural, and external service requirements.
- Provide professional supervision of the pharmacy team.
- Provide advice and guidance to pharmacy colleagues to ensure skills and experience are shared and used to the benefit of the pharmacy and individuals.
- Undertake pharmacy audits as required.
- Support the Pharmacy Superintendent and Pharmacy Operations Manager in the development and improvement of Standard Operating Procedures and all other aspects of HealthNet's Pharmacy Homecare Service.
- Proactively build and maintain effective working relationships with referring centre prescribers, pharmacists, and nurses, taking professional responsibility for the development and enhancement of services provided.
- Provide professional support and advice about medicines to HealthNet's nursing team.
- Carry out any reasonable request given by appropriate persons.
Requirements
- MPharm Degree or equivalent qualification that meets the Criteria for initial registration as a pharmacist with the General Pharmaceutical Council.
- Registration with the General Pharmaceutical Council.
- Experience in a Homecare Pharmacy setting (advantageous).
- Excellent communication and stakeholder management skills with strong written and oral skills.
- Strong judgement skills and decision-making ability.
