About the Job

We are looking for an experienced Nurse Practitioner for Diabetes Management to join our dedicated team at the Southern Health and Social Care Trust. In this role, you will be responsible for providing high-quality patient care, conducting thorough assessments, and developing individualized care plans.

Key Responsibilities

We offer a competitive salary of £34,000 - £45,667 per annum, depending on experience, and a range of benefits, including flexible working arrangements and ongoing training and development opportunities.
