Diabetes Specialist Care Coordinator

As a Diabetes Specialist Care Coordinator, you will be responsible for providing high-quality nursing care to patients with diabetes. Your role will involve conducting comprehensive assessments of patients' physical and emotional needs, developing and implementing personalized care plans to improve patients' health outcomes, and collaborating with healthcare teams to ensure seamless care coordination.
