About the Role

We are seeking a highly motivated and experienced Healthcare Assistant to join our Peripatetic Team. As a key member of our community care team, you will work closely with patients, carers, and healthcare professionals to provide high-quality care and support.

Key Responsibilities
  • Work collaboratively with the Peripatetic Insulin Nurse to ensure seamless care delivery
  • Support patients with insulin administration, wound care, and other community-based services
  • Contribute to the development and implementation of care plans, ensuring patient-centered care
  • Communicate effectively with patients, carers, and healthcare professionals to ensure seamless care delivery
  • Participate in health promotion and education activities to support patient empowerment
Person Specification
  • NVQ 2 Health and Social Care or equivalent knowledge and experience
  • Experience of working within a clinical setting, preferably in a community-based role
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals
  • Ability to work flexibly, including evenings and weekends, to meet patient needs
  • Valid driving license and access to a vehicle
