Job Description

This role involves working with vulnerable individuals, providing them with support and guidance to help them achieve their goals. As a Support Worker, you will deliver structured support and risk management, engaging with customers to develop bespoke support plans that meet their unique needs.

You will work collaboratively with key stakeholders, including family members and other support providers, to ensure a comprehensive approach to customer care. The ideal candidate will have experience of working with vulnerable people, a strong understanding of the barriers faced by diverse customer groups, and the ability to work flexibly to meet customer and business needs.

You will be responsible for leading on co-producing bespoke support and move-on plans, organising regular planned reviews of support and risk plans, and assisting customers with day-to-day support and tenancy-related matters. You will also identify and promote opportunities for employment, education, and training, and support customers to remove barriers to accessing these opportunities.
