Your Responsibilities

  • Reviewing and updating resident care plans to ensure individualised care
  • Administering and ordering medication in accordance with guidelines
  • Liaising with GP's, district nursing teams, and family members to ensure seamless care coordination

About You

We are looking for an experienced Senior Care Coordinator who possesses excellent communication and organisational skills. You should have a strong understanding of care coordination principles and be able to work effectively with multiple stakeholders.
