Community Care Coordinator

We are seeking a highly skilled and experienced Field Care Supervisor to join our team at City and County Healthcare Group Ltd. As a Field Care Supervisor, you will be responsible for ensuring the highest standards of care support services are delivered in the community. You will work closely with care coordinators, care workers, and other stakeholders to achieve this goal.
Responsibilities- Work with care coordinators to plan realistic and effective care worker rotas.
- Carry out regular reviews of individual services to ensure they meet the needs of service users.
- Shadow new care workers to ensure their competence.
- Participate in multi-disciplinary reviews of service users' needs.
To be successful in this role, you will need experience in supervising and instructing care workers, assessing and reviewing individual services, and participating in multi-disciplinary reviews. You will also need excellent communication and interpersonal skills.
What We Offer- A competitive salary of £32,000 - £37,000 per annum.
