About the Role

We are seeking a highly motivated and experienced Social Worker to join our Neighbourhood Team in Bolton. As a Social Worker, you will be responsible for providing high-quality care and support to adults and older adults in the west/central north area.

You will work alongside a multi-disciplinary team to provide a joined-up approach to meeting the needs of individuals within our neighbourhoods. Your key responsibilities will include carrying out assessments of need under the Care Act 2014, devising and implementing strengths-based support plans, and supporting service users to access universal and voluntary services.

You will also be expected to carry out assessments, carers assessments, and formulate support plans under the Care Act 2014. Knowledge, experience, and the ability to adhere to the principles of the Mental Capacity Act 2005 and other Social Work legislation and practice guidance are essential.

The role includes covering a busy duty system on a rota basis, taking referrals, and offering information and advice at point of contact. You will have the opportunity to work with a supportive team and contribute to the development and implementation of successful outcomes for Bolton residents.

Key Responsibilities

  • Carry out assessments of need under the Care Act 2014
  • Devising and implementing strengths-based support plans
  • Supporting service users to access universal and voluntary services
  • Carrying out assessments, carers assessments, and formulating support plans under the Care Act 2014
  • Working with a multi-disciplinary team to provide a joined-up approach to meeting the needs of individuals within our neighbourhoods

About You

We are looking for an enthusiastic and motivated individual who has a passion for Social Work practice and experience of working with adults in a community setting. You will have the ability to develop, maintain, and improve relationships with service users and carers, as well as the attributes required to participate in inter-disciplinary team working.

The posts are subject to enhanced clearance from the Disclosure and Barring Service and registration with Social Work England. We welcome applications from Student Social Workers who are in their final placement and are due to qualify in the next 6 months.
