About the Role

We are seeking a highly skilled and compassionate Service Coordinator to join our national award-winning support team in Bolton. As a Service Coordinator, you will play a vital role in providing support to individuals with multiple and complex needs, helping them to develop the skills needed to move on into independent living or permanent housing.

Key Responsibilities

  • Manage a caseload of service users, providing person-centred support packages tailored to their individual needs.
  • Work collaboratively with colleagues to ensure a 24-hour service, contributing to the staffing of a dynamic and supportive team.
  • Develop and maintain relationships with service users, empowering them to take control of their lives and make informed decisions.
  • Record and report case notes via an online Customer Relationship Management system, ensuring accurate and timely documentation.
  • Identify, monitor, and report safeguarding concerns, upholding the highest standards of care and support.

About You

We are looking for a Service Coordinator with:

  • Experience of managing a caseload of service users, with a strong understanding of issues related to vulnerable individuals.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users and colleagues.
  • Knowledge of safeguarding policies and procedures, with the ability to identify and report concerns.
  • A flexible and resilient approach, with the ability to work in a fast-paced environment.
  • A commitment to upholding the values and ethos of Adullam Homes, with a passion for delivering high-quality care and support.
