About the Role

We are seeking a highly skilled and compassionate Stratified Follow-up Navigator to join our team at Lewisham and Greenwich NHS Trust. As a key member of our cancer care team, you will play a vital role in providing exceptional support and guidance to patients and their families.

The ideal candidate will have a strong understanding of cancer care pathways and be able to navigate complex systems with ease. You will be responsible for navigating patients through the stratified follow-up process, ensuring that they receive the necessary care and support. Your excellent communication skills and empathetic nature will enable you to build strong relationships with patients and healthcare professionals alike.

Key Responsibilities:
