Job Summary

We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our team at Pennine Care NHS FT. As a Care Coordinator, you will play a vital role in providing comprehensive mental health services to adults of working age who experience moderate to severe mental health problems within the community.

Main Responsibilities
  • Work collaboratively across a range of disciplines to promote a 'needs-based' approach to service users.
  • Promote an integrated approach to service delivery.
  • Develop clear relapse recognition and prevention plans and share with the multidisciplinary team, service users, and carers as appropriate.
  • Develop meaningful engagement with individuals who are often referred to as 'difficult to engage' and their families.
Key Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with current registration with the appropriate regulatory body.
  • Substantial experience of relevant post-registration practice, including managing a caseload and working in a multi-disciplinary team setting.
  • Excellent written and interpersonal communication skills, with the ability to engage with and provide therapeutic interventions for individuals with complex mental health needs.
