About the Role

We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our team as a Care Coordinator. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to individuals with mental health needs.

Key Responsibilities
  • Provide comprehensive mental health services to adults of working age with moderate to severe mental health problems.
  • Work collaboratively with a range of disciplines to promote a 'needs-based' approach to service users.
  • Develop clear relapse recognition and prevention plans, and share with the multidisciplinary team, service users, and carers as appropriate.
  • Engage with individuals who are often referred to as 'difficult to engage' and their families.
Main Duties

The successful applicant will work effectively with adult service users with mental health needs across the community mental health team, predominantly in the active care coordination function. They will manage a defined caseload and be highly skilled and competent in conducting comprehensive holistic health and social needs assessments and risk assessments, developing outcome-based needs-led care plans, facilitating the implementation of the agreed plan of care, and evaluating its effectiveness using evidence-based outcome measures with a focus on recovery and social.

Service/Ward/Department Summary

The community mental health team works primarily with working-age adults who have a diagnosis of a serious mental illness and/or complex mental health difficulties. The care pathway includes a holistic assessment of need/risk, formulation, and delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning.
