Job Overview

The successful candidate will work in a multi-disciplinary team setting, collaborating with mental health nurses, social workers, CBT therapists, and support workers to provide high-quality care to individuals experiencing first episode psychosis.

Main Duties

As a Care Coordinator, you will be responsible for managing a defined caseload of individuals, working with them to meet their identified needs using outcome measures and a recovery approach. You will also facilitate discharge from secondary mental health services or referral as appropriate.

Additionally, you will contribute to duty and clinic work, participate in team development issues, and engage with individuals and carers to provide therapeutic interventions.
