Castleton Health Centre is seeking an experienced and skilled Advanced Nurse Practitioner to join our primary care team. As an Advanced Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to our community.
Key Responsibilities:
- Assess and triage patients, including same-day triage, and provide definitive treatment or make necessary referrals to other members of the primary care team.
- Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology, taking appropriate action.
- Use complex decision-making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
- Take a personalized care approach and population-centred care approach to enable shared decision-making with patients.
- Provide direct access to a nurse practitioner for the general practice population, both in the practice and over the telephone.
- Make home visits to those in need where required.
- Make professionally autonomous decisions for which you are accountable.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
- Prescribe safe, effective, and appropriate medication as defined by the current legislative framework in accordance with the practice prescribing formulary.
- Provide safe, evidence-based, cost-effective, individualized patient care.
- Offer a holistic service to patients and their families, developing an ongoing plan of care/support, with an emphasis on prevention and self-care.
- Promote health and well-being through the use of health promotion, health education, screening, and therapeutic communication skills.
- Refer patients directly to other services/agencies as appropriate.
- Contribute to the practice achieving its quality targets, such as QOF and IIF, to sustain high standards of patient care and service delivery.
- Help develop and set up new patient services and participate in initiatives to improve existing patient services.
