Job Summary

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Tredegar Health Centre. As an autonomous healthcare professional, you will work closely with our multidisciplinary team to provide high-quality care and treatment to patients with a range of health needs.

Main Responsibilities
  • Diagnose and manage patients with acute and chronic conditions, including those with undiagnosed or undifferentiated diagnoses.
  • Prescribe and dispense medications as necessary, and refer patients to secondary care or organize emergency admissions when required.
  • Conduct face-to-face, telephone, and video consultations with patients, and make home visits as needed.
  • Manage a caseload of patients, ensuring timely and effective care and treatment.
  • Collaborate with other healthcare professionals, including GPs, nurses, and clinical pharmacists, to provide comprehensive care and support.
  • Develop and implement care plans, and monitor patient progress and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures.

Requirements
  • Registered Nurse with a Master's degree or Postgraduate Diploma in Advanced Practice, or equivalent qualification.
  • Qualified Independent Nurse Prescriber, with experience in prescribing and managing medications.
  • Minor Illness Qualification, with experience in managing acute and chronic conditions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong clinical skills, with the ability to diagnose, manage, and treat patients with a range of health needs.
  • Ability to work autonomously, with minimal supervision, and to make decisions in a fast-paced and dynamic environment.
