Main Responsibilities- Diagnose and manage patients with acute and chronic conditions, including those with undiagnosed or undifferentiated diagnoses.
- Prescribe and dispense medications as necessary, and refer patients to secondary care or organize emergency admissions when required.
- Conduct face-to-face, telephone, and video consultations with patients, and make home visits as needed.
- Manage a caseload of patients, ensuring timely and effective care and treatment.
- Collaborate with other healthcare professionals, including GPs, nurses, and clinical pharmacists, to provide comprehensive care and support.

Requirements- Registered Nurse with a Master's degree or postgraduate diploma in Advanced Practice Qualification.
