We are seeking an experienced Advanced Nurse Practitioner to join our team at Tredegar Health Centre. As an autonomous practitioner, you will work closely with our multidisciplinary team to provide high-quality care to patients with a range of health needs.
Main Responsibilities- Diagnose and manage patients with acute and chronic conditions, including those with undiagnosed undifferentiated diagnoses.
- Prescribe and dispense medications as necessary, ensuring the highest standards of care for all patients.
- Work collaboratively with GPs, Nurse Practitioners, Clinical Pharmacists, and Nurses to provide comprehensive care to patients.
- Manage a caseload of patients, ensuring timely and effective care, and making referrals to secondary care as needed.
- Conduct face-to-face, telephone, and video consultations, as well as home visits, to provide care and support to patients.
