Job Summary

We are seeking an experienced Advanced Nurse Practitioner to join our team at Tredegar Health Centre. As an autonomous practitioner, you will work closely with our multidisciplinary team to provide high-quality care to patients with a range of health needs.

Main Responsibilities
  • Diagnose, manage, and prescribe treatment for patients with acute and chronic conditions.
  • Work independently to manage a caseload and make decisions about patient care.
  • Collaborate with other healthcare professionals to ensure seamless care and treatment.
  • Develop and implement care plans to meet the needs of patients with complex health conditions.
  • Provide education and support to patients and their families to promote health and well-being.

Person Specification
  • Registered Nurse with Nursing and Midwifery Council registration.
  • Masters degree or Postgraduate Diploma in Advanced Practice or equivalent qualification.
  • Qualified Independent Nurse Prescriber.
  • Minor Illness Qualification.
