About the Role
We are seeking an experienced Advanced Nurse Practitioner to join our team at Tredegar Health Centre. As an autonomous practitioner, you will work independently to diagnose, manage, prescribe, and discharge patients with various health conditions.
Key Responsibilities
- Provide high-quality patient care, including consultations, home visits, and telephone consultations.
- Manage acute and chronic presentations, including ordering investigations and referring to secondary care as needed.
- Prescribe medications and treatments, ensuring the highest standards of care for patients.
- Collaborate with the multidisciplinary team, including GPs, nurses, and pharmacists, to provide comprehensive care.
- Manage a caseload, prioritizing tasks and ensuring timely completion of duties.
Person Specification
We are looking for a qualified and experienced Advanced Nurse Practitioner with a Master's degree or postgraduate diploma in Advanced Practice. You should be a registered nurse with the Nursing and Midwifery Council and hold a Minor Illness qualification. As an Independent Nurse Prescriber, you will be able to prescribe medications and treatments autonomously.
